Financial Statement Booklet
Information
NOTE: Submission of the Board’s Financial Statement Booklet is required for licensing of Prime contractors. The booklet must be prepared by a CPA not connected with the applicant. See Booklet—page 1 for full instructions.
The Board’s Financial Statement Booklet is available online in MicroSoft Excel format
(.xls).
PLEASE NOTE:
LEFT Click on the Download File link at bottom of page to
either display the file within your browser window or to download the file (it will
automatically download depending on your browser version). It
is recommended you RIGHT CLICK and SAVE the file instead
(as stated below).

Read below for different browser specifics:
- Microsoft Internet Explorer: After clicking on the Download File link at bottom
of the page with the RIGHT mouse button, select the "Save Target As.."
option to download the file. A save window will appear allowing you to save the file
to your computer.
- Netscape: After clicking the on the Download File link at
bottom of the page with the RIGHT mouse button, select the "Save Link
As.." option to download the file. A save window will appear allowing
you to save the file to your computer.
